Required enrollment documents
To ensure a smooth enrollment process at Lemonade Family Child Care, we kindly ask all parents to download and complete the required forms. These forms are crucial for your child’s safety and our compliance with regulations. They include the Identification and Emergency Information Form, Consent for Emergency Medical Treatment Form, Family Child Care Home Notification of Parents’ Rights Form, Sunscreen Permission Form, and Photo Permission Form. Below you’ll find links to download each form. Completing these in advance helps us prepare for your child’s arrival and ensures that we can provide the best care from day one.
